According to his death certificate, Lewis's birthplace was Almond, New York (though the Family Bible lists it as Andover). He was widowed and a retired farmer. He resided at 18 Wells Street in Hornell, N.Y., the home of his son, for 5 years and 4 months, a period of time that coincides with the death of his wife. His cause of death was listed as cardiac insufficiency with probable bronchio pneumonia and chronic diarrhea a contributing factor, an affliction he reportedly suffered for a year prior to his death. C.P. Kaple of 18 Wells Street, Hornell, N.Y. was the informant.
